This certificate is considered void if sold, bartered or purchased. This certificate is valid for a grocery rebate registration form offering you the opportunity to receive $300 in grocery rebates. The registration form allows you to get $300 in groceries in increments of a $25 rebate for every $300 you spend in groceries monthly. You can only receive one $25 rebate a month. The customer is required to purchase the groceries with a debit/credit card with the last 4 digits matching on each receipt. We do this to prevent fraud; this clearly identifies the person purchasing the groceries. In order for you to receive the registration form to become a member in this grocery rebate program is just a one-time $35.00 processing fee. The redemption fee will be fully refundable only if you complete the grocery rebate program in its entirety. The refundable redemption fee will be refunded by the redemption company and not the fulfillment company. When you receive the registration form you will need to register by choosing which grocery store brand you will want to receive your $300 in grocery rebates. You can only choose one of the participating grocery brands listed. Once registered you will be required to send in $300 in grocery receipts from the grocery store you pre selected once a month to prove that you spent $300 in groceries. You will then receive a $25 rebate that can be used for groceries at the grocery store you pre selected. Along with submitting your Acknowledgement/Registration form by certified mail, you must provide a self-addressed, stamped envelope with 44 cents for return postage to receive your first Rebate Coupon (and for each coupon thereafter) to begin your rebate process. You can only redeem a maximum of $25 per month in rebates. The expiration date on this certificate does not pertain to the rebate program length. Once redeemed the rebate program will be good for a minimum of 12 months. You must redeem this certificate by the expiration date printed on this certificate. This offer is only good for United States and Canada Residents. This offer is non-refundable until the completion of the program. Only one grocery certificate redeemable per household per year. Lost or stolen registration forms will not be replaced. This certificate has no cash surrender value, void if discontinued and where prohibited by law. The terms of this offer are subject to change without notice. Failure to follow all the terms and conditions will null void your rebate program and your $35 will not be refundable if this happens.
